Please join members of the St Louis Community Health Worker Coalition (STLCHWC) as they share progress, to date, leading a social impact effort within the region sponsored by the CDC - to build resilient communities. Members of the coalition will not only share the advancements and achievements of the work, but also opportunities for partnership and engagement. Finally, presenters from the Coalition will share an overview of their organization, the current expansion the Community Health Worker certification course, avenues for patient resource connections for health related social needs through the Coalition, as well as the supportive professional network that is available to your community- facing team members, like CHWs. 

Objectives:

  • Understand the progress and expected deliverables of the St. Louis Community Health Worker Coalition
  • Describe the expansion of St. Louis Community College's Community Health Worker Certification
  • Create pathways for engagement and partnership across institutions and with surrounding neighbors
  • Identify the need for a network of CHWs for your community facing team members and a path for client care transitions around social determinants of health
